Certified and Emergency Certified Substitutes
  • Certified Substitute:
  • A certified substitute holds a teaching certificate or statement of eligibility and may substitute for a teacher.
  • In order for you to be added to a substitute list, all paperwork must be completed by the 15th of the previous month. This includes a completed background check and physical.

     1. Complete the online employment application here.  Click Apply for this Position on the top right corner of the screen.If you have not completed an application through School Recruiter before you will need to create an account. If you have applied to any position using School Recruiter previously you will be able to sign in or transfer your account from another school district. You will receive an email confirmation once you have submitted your application.

  •   2. Once you have received your email confirmation you will need to complete the substitute packet of information that is available below. Please be sure to read through the notes below regarding the specific forms. You will need to print the forms; complete the forms and once they are all complete you may return them to Teresa Osbourn at Central Office, 152 E. Martin Luther King Blvd, Danville, KY 40422.

  •  § Copy of Teaching Certificate
  •  § Copy of Official Transcripts
  •  § Background Check Form (this is a form to complete your fingerprint card, you will be fingerprinted when you return your paperwork, please bring $34.75 in order to process the fingerprint card. We accept cash, checks or money orders. Cash must be the exact amount. All checks/money orders should be made payable to Danville Board of Education.)

    § Direct Deposit Enrollment (attach a voided check)

    § W-4 Form

    § K-4 Form

    § I-9 Form (attach a copy of your social security card and driver's license, or other acceptable documents) 

    § Worker's Compensation Form

    § Safety Procedures Manual (only print and complete the signature page)
  • § Staff Internet Policy

    § Statement Concerning Social Security

    § Professional Code of Ethics (this is for you to review, nothing needs to be completed and returned)
  •  § Medical Exam Form (you will need to take this form to a doctor of your choice and have them complete the form after conducting a physical examination. You are responsible for any and all costs related to this physical examination.)
  • § Health Insurance Part A Notice (sign and return the second page only)
    § Bloodborne Pathogen Online Training (complete the online training and print out your certificate to include with your paperwork)
  • § Promoting Positive Behavior Training (complete the online training and print out your certificate to include with your paperwork)

§ You will need to complete a TC-4 application for one-year Emergency Substitute Certification. (Emergency Substitutes must secure an emergency substitute certificate for each district in which they wish to work.)
§ You will receive an email from EPSB that Danville Schools has initiated the process for you to complete the TC-4 application:
    o Click on the link provided in e-mail notification from EPSB/Danville Schools and create an account by clicking on the “Log in to EPSB” button on the lower left of the screen.
    o If necessary, create your new account on the right side of the log-in page by clicking on the “New User? Create Account Here” button.
    o Be sure to make note of your User ID and password for future reference.
    o Make sure all of your personal information is correct when you establish your EPSB account.
    o Once you are logged in, click on “My Application Status-View Details” on the lower left of the screen.
    o You will then see your application (TC-4) and you should click on the live link to complete the process on your pending application.
    o Verify your personal information and click “Save and Next” o Add any previously held out-of-state teaching certificate (if applicable), Click “Next”
    o Answer all Character & Fitness questions (each question must be answered by clicking on the small pencil in the “Edit” column)
    o Enter password to digitally sign and submit the application o After your application is submitted: § If your TC-4 application process is successful, you will receive a live link to print your Emergency Substitute Certificate.
§ Applicants will receive an email from EPSB when a Character & Fitness review is completed and approved. (Supplying false information on the Character & Fitness form will result in a case being opened by the Board on the applicant.)
